I, as the parent/legal guardian of the above named child (hereafter referred to as "my child"), hereby acknowledge and agree to the following terms and conditions for my child to participate in the children's sewing class offered by Sincere Studio.

 

1. Assumption of Risk: I understand that participation in the sewing class involves certain risks, including but not limited to, needle pricks, cuts, burns, and other injuries associated with sewing and the use of sewing equipment. I acknowledge that Sincere Studio has taken reasonable precautions to minimize such risks, but I understand that accidents can still occur.

 

2. Release of Liability: In consideration of my child being permitted to participate in the sewing class, I hereby release and discharge Sincere Studio, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child , or to any property belonging to my child, whether caused by the negligence of Sincere Studio or otherwise, while participating in the sewing class.

 

3. Indemnification: I agree to indemnify and hold harmless Sincere Studio, its directors, officers, employees, volunteers, and agents from and against any and all liabilities, claims, demands, actions, or causes of action, including attorney's fees and costs, arising out of or related to my child's participation in the sewing class.

 

4. Emergency Medical Treatment: In the event of any illness, injury, or medical emergency involving my child during the sewing class, I authorize Sincere Studio and its representatives to administer first aid and seek emergency medical treatment as deemed necessary. I understand that I will be responsible for any expenses incurred for such medical treatment and have appropriate health insurance for my child.

 

5. Photo/Video Release: I grant Sincere Studio the irrevocable right and permission to photograph and/or videotape my child during the sewing class and to use such photographs and/or videos for promotional and educational purposes

 

6. Compliance with Rules and Instructions: I understand that my child is expected to comply with all rules and instructions provided by the instructors and staff of Sincere Studio during the sewing class. I agree to ensure that my child follows all safety guidelines and procedures.

 

7. Governing Law: This waiver and release shall be governed by and construed in accordance with the laws of the state of Oregon.

 

I have read this waiver and release, fully understand its terms, and voluntarily agree to its contents. I acknowledge that I have the legal authority to sign this agreement on behalf of my child.
